Talk with your doctor if you think you may have RLS. Often, there's no known cause for RLS. Researchers suspect the condition may be caused by an imbalance of the brain chemical dopamine, which sends messages to control muscle movement. Sometimes RLS runs in families, especially if the condition starts before age Researchers have identified sites on the chromosomes where genes for RLS may be present. Pregnancy or hormonal changes may temporarily worsen RLS signs and symptoms.
Some women get RLS for the first time during pregnancy, especially during their last trimester. However, symptoms usually disappear after delivery. RLS can develop at any age, even during childhood.
The disorder is more common with increasing age and more common in women than in men. RLS usually isn't related to a serious underlying medical problem. However, it sometimes accompanies other conditions, such as:. Although RLS doesn't lead to other serious conditions, symptoms can range from barely bothersome to incapacitating. Many people with RLS find it difficult to fall or stay asleep. Severe RLS can cause marked impairment in life quality and can result in depression.
Insomnia may lead to excessive daytime drowsiness, but RLS may interfere with napping. Restless legs syndrome care at Mayo Clinic. Mayo Clinic does not endorse companies or products.
To our knowledge, this is the first case of restless legs syndrome with arm restlessness as the initial symptom in an otherwise neurologically normal person.
Conclusion Restless legs syndrome may initially exhibit upper extremity symptoms. They often disrupt sleep, producing sleep-onset and sleep-maintenance insomnia.
If patients are symptomatic in the day, they describe worsening symptoms as the day progresses. Excessive daytime somnolence has also been reported in a substantial number of patients. We report a case of RLS with arm restlessness as the initial symptom in a patient with no other neurologic abnormalities.
A year-old man had a 1-year history of uncomfortable sensations in his upper extremities. He reported "twitching and wiggling" sensations in his arms that were worse at rest, and felt the need to move his arms. The symptoms were bilaterally symmetric. Walking decreased the symptoms. Initially, his symptoms were present only in the very early morning, awakening him during sleep.
Because of excessive daytime somnolence, he took a nap in the afternoon and fell asleep in the early evening while reading or watching television. His sleep was not refreshing. He had a history of coronary artery disease, coronary bypass surgery, benign prostatic hypertrophy, hiatal hernia, and pulmonary tuberculosis remotely in the past. His medications included furosemide, potassium chloride, digoxin, lovastatin, aspirin, amiodarone, terazosin, a multivitamin, and vitamin E. He did not smoke or use excessive caffeine.
Substantial alcohol use had ceased years before the onset of arm restlessness. There was no family history of movement disorders or sleep disorders. Results of laboratory tests, including serum chemistry, complete blood cell count, thyroid tests, iron studies, and vitamin B 12 levels, were normal.
A nocturnal polysomnogram demonstrated periodic leg movements Figure 1 , poor sleep efficiency, and disruption of sleep architecture with excessive arousals Table 1. A 4-minute sample from the patient's nocturnal polysomongram demonstrates periodic leg movements thin arrow in channel 7 R.
LEG and arousals block arrow in channel 4 O2-M1. The numbers at the bottom indicate sleep stage. The arm restlessness became more pronounced in the evening as his illness progressed. Symptoms partially responded to tramadol although it soon lost its effectiveness. The main symptom of restless legs syndrome is an overwhelming urge to move your legs. It can also cause an unpleasant crawling or creeping sensation in the feet, calves and thighs.
The sensation is often worse in the evening or at night. Occasionally, the arms are affected, too. Restless legs syndrome is also associated with involuntary jerking of the legs and arms, known as periodic limb movements in sleep PLMS. Some people have the symptoms of restless legs syndrome occasionally, while others have them every day. The symptoms can vary from mild to severe. In severe cases, restless legs syndrome can be very distressing and disrupt a person's daily activities.
In the majority of cases, there's no obvious cause of restless legs syndrome. Our staff will be happy to help allocate you to the first available appointment slot that best fits your convenience.
